Communication passport templates

Communication passports are a fantastic way to provide a snapshot of the key information about an individual with communication needs, to help others to get to know them better. They allow for practical information to be provided for family, friends, professionals, and others they may meet. The Language Box templates provide space to provide the following information:

- A cover page, with the ability to include a photograph and personalise
- Special people, my family, my friends
- Things that I like
- Things that I don't like
- How I communicate
- I might need your help with
- If I am upset, frustrated or angry I might
- Anything else key you need to know about me
